Practice Transformation as a Driver for Interprofessional Practice and Education

Theme: Optimizing the Interprofessional Clinical Learning Environment

This interactive session includes a panel of speakers covering emerging state and national initiatives, projects, data, and the partnerships forming to achieve practice transformation. The presenters will highlight the work of Healthier Washington and specifically share the work of the practice transformation efforts of a regional accountable community of health. Facilitators will engage the audience in a discussion regarding the community, regional and statewide activities impacting health workforce and the efforts to align with the priorities of industry and community.
Participants will gain an understanding of how current and emerging health care policies are impacting community health care practice transformation efforts. Specific examples will be shared of organizational and community practice transformation efforts to address emergency room diversion, care transitions, behavioral health/primary care integration, and care coordination for patients with chronic care conditions.

Interprofessional Continuing Education

This activity has been planned and implemented by the National Center for Interprofessional Practice and Education. In support of improving patient care, The National Center for Interprofessional Practice and Education is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC) and the Association of Social Work Boards (ASWB) to provide continuing education for the healthcare team.
