Ms. Sam Werdel has over 20 years of healthcare administration experience in academic, for profit and nonprofit health systems. She has also consulted for healthcare entities over the last eight years, providing practice transformation, EHR support, financial review, and process improvement services. Most recently, she is the Practice Transformation Director for Greater Columbia Accountable Community of Health (GCACH). Her team at GCACH are implementing standardized practice transformation strategies for health systems and community health centers, supporting Healthier Washington’s objectives of bi-directional integration, opioid crisis, transitional care, and chronic disease management.

The presenters will highlight the work of Healthier Washington and specifically share the work of the practice transformation efforts of a regional accountable community of health.
