Collaborative Learning for Integrating Primary Care and Behavioral Health

Working across state agencies, the Minnesota Department of Health and Minnesota Department of Human Services are engaging primary care and behavioral health providers to redefine health and redesign care. At the center of this process is creating a collaborative learning environment where providers and their partners continually improve to deliver whole person care by integrating across primary care, behavioral health, and social services. Participants at this interactive “show and tell” learning session will enhance knowledge and skills for:
• Identifying integrated learning topics for whole-person care
• Delivering engaging, accessible content that stakeholders can use
• Reinforcing learning through ongoing support and supervisor engagement
• Collaborating with community and agency partners to design and deliver learning
• Partnering with stakeholders to create peer-based learning opportunities
