David Kurtzon, MPH, is a Planner Principal with the Minnesota Department of Health's Health Care Homes (HCH) program. He is instrumental in creating e-learning infrastructure and online learning opportunities for primary care clinics and community partners who are working together to redefine health and redesign care. David collaborates with colleagues in the collection and analysis of learning-related data, implementation of live trainings, and development of the HCH learning strategy. He serves as a site administrator for the Department of Health's learning management systen and as division system administrator for Health Policy.

Working across state agencies, the Minnesota Department of Health and Minnesota Department of Human Services are engaging primary care and behavioral health providers to redefine health and redesign care. At the center of this process is creating a collaborative learning environment where providers and their partners continually improve to deliver whole person care by integrating across primary care, behavioral health, and social services.
