Tracey Smith, DNP, PHCNS-BC, MS associate research professor is the Excecutive Director of the Office of Community Initiatives and Complex Care and the Director of Population Health Integration at Southern Illinois University School of Medicine and for the Family Medicine FQHC site. She has been involved in healthcare in numerous roles allowing her to build skills to engage multiple types of health care providers and patients. For the past 20 years she has been employed at Southern Illinois University School of Medicine always functioning in a capacity to provide education to all levels of medical students especially in the areas of population health and prevention, health disparities, community and patient engagement, and conduct research with a focus on engagement, health literacy, and social disparities. She is the director of one of four national student hotspotting HUBs for the Camden Coalition for Student Hotspotting.

Workshop Description:Academic and community partnerships have the potential to create win-win scenarios for interprofessional learners, patients, and community members. This session will provide an opportunity for attendees to learn from the experience of the four Interprofessional Student Hotspotting Learning Collaborative Hubs, supported by the Camden Coalition and National Center for Complex Health and Social Needs.
