Lynda Heinig is a creative writer who is also a bachelor prepared registered nurse having now worked for Trinity Health for 38 years. She has worked in areas of Med-Surgery, Home care, Surgery General team, and currently is a Care Manager for the SEMI ACO/CIN. Lyndas current job focuses on educating patient population regarding chronic health problems, managing those with mutliple co-morbidities to decrease exacerbations and readmissions.

Re-admissions continue to plague our healthcare systems, and confusing discharge processes can be a factor in unintentional errors and increased ED/hospital utilization. To address these issues, I-MPACT, a Blue Cross Blue Shield of Michigan collaborative quality initiative, assembles hospitals and physician organizations to work as local “clusters” with patient participants to develop viable interventions.
